If your child is not eligible for free school meals, you can apply for a paid place to our Fuel4Fun school camps. Book a paid place on the camp here.
Unique Pupil Number
If you do not enter the correct UPN here, your form will not be sent to our team for processing. Your UPN number is the Unique Pupil Number given to your child when they enter mainstream school. Please contact your school if you do not know your UPN number and then come back to complete this form.

As part of the Fuel4Fun holiday camp, your child is provided with a food hamper and recipes with enough food to feed your family for 5 days. Food hampers will be delivered to the address listed above so please ensure all the information is correct.
Swimming kits will be needed at all holiday camps with the exception of gymnastics.

Active Tameside would like to take your child's photograph at the Holiday Camp for promotional purposes. These images may appear as part of our publicity campaign in and around the borough on social media, website, local media or as part of promotional material for the organisation. To comply with the Data Protection Act 1998 and GDPR guidelines we need written permission before we take any photograph or recordings. Anyone under the age of 18 must be given consent by a parent or guardian. Active Tameside recognises the need to ensure the welfare and safety of children. Active Tameside will take all steps to ensure these images are used solely for the purpose they intended. If you become aware that these images are being used inappropriately you should inform the manager immediately. If you agree please tick the appropriate boxes below. By ticking these boxes and submitting the form you are agreeing to the above statements and this information can be kept up to 3 years from date of submission.
